This program is for patients who need to decrease their risk factors, or who have recently had:
- a heart attack or angina
- a coronary artery bypass surgery (CABG)
- a percutaneous transluminal coronary angioplasty (PTCA)
- heart valve surgery
- heart failure
- peripheral artery disease

Our multidisciplinary team includes a cardiologist or internist, registered nurses, dietitian, occupational therapist, social worker, psychologist and clerical staff.
Referrals
Patients will need a physician referral to access the cardiac rehabilitation program.
