Request health information or health records
The Access and Disclosure team in Health Records specializes in processing requests for health information according to Alberta’s Health Information Act.
To request health records, please follow the steps below:
- Fill out the Health Information Access Request form, instructions are on page 2.
- Mail, fax or drop off your completed and signed request form and copy of ID to the attention of Access & Disclosure, Health Records at a Covenant Health hospital or health care centre where you received treatment.

Fax number: 1.403.762.4193 - To request health information on behalf of another person, please provide proof of authority to request the information. (eg. written authorization, Authorizing a Representative form, Guardianship, Power of Attorney, etc.)
- Once we have received your request, you will be contacted should we require additional information. In accordance with the Health Information Act, you may expect a response back from us within 30 days, and we make every effort to respond sooner.
