Geriatric Community Rehabilitation (GCR)
- GCR specializes in an interdisciplinary approach to geriatric assessment and rehabilitation in a community setting.
- Clients attend for up to 24 sessions (3 months).
- This program benefits those who have recently experienced changes in their functional ability due to an illness or other medical event (such as a stroke, post-orthopedic injury or surgery)
- The disciplinary team includes a physiotherapist, occupational therapist, recreation therapist, pharmacist, dietitian, geriatrician and registered nurse.
Bridges
- Bridges is a day program providing many recreational activities, supervised group exercise and access to the the various health care and rehabilitation professionals as needed.
- This program benefits seniors who have the potential to decondition due to chronic conditions, require maintenance after completing GCR, need more socialization or whose caregiver requires respite.
- It offers a ‘bridge’ from home to supportive living with the aim of maintaining independence and delaying or preventing a move to a care facility.
- Clients attend for three hours per day for up to three days per week.
Adult Day Program
- This program benefits seniors living with more advanced cognitive challenges from dementia, with or without physical disabilities, in their own home. It provides a safe environment to socialize and participate in various activities and offers respite for caregivers.
- Clients attend for six hours a day up to twice per week.
Referrals
Physician referral is required. Home Care Case Coordinators or other programs/disciplines can refer with approval from a family physician.
