Community Day Support Program
The Community Day Support Program serves clients through our medical rehabilitation program. We serve adults with physical, emotional and/or social limitations who are living in the community. Services include nursing assessment/monitoring and rehabilitation (physiotherapy, occupational therapy and recreational therapy).
Admission criteria
To be admitted to the day program, a client must:
- Require reassessment, rehabilitation or maintenance
- Live in the geographical area serviced by Alberta Health
- Live in the community or be in the process of being discharged from a care centre or hospital
- Have contact with a physician in the community
- Be motivated to become involved in the program and committed to attend on a regular basis
- Be able to function in a group and doesn't require continual one-on-one supervision
- Benefit from the services provided by the program
- Not demonstrate behavior which may be harmful to him/her or others
- Not require a mechanical lift for transfers
- Have care needs that can be met by program staff

- Help clients continue living in their homes in the community as long as possible

Services
- Nursing care and supervision

- Socialization and therapeutic recreation
Fee
- $10 per visit as set by Alberta Health Services
- We ask that our clients pre-authorize payments for monthly billing.
- Clients are responsible for the fee of each scheduled visit day unless admitted to hospital.
- Clients are allowed two weeks per calendar year for holidays; any holiday exceeding the two weeks will be billable.
Transportation
We can help coordinate transportation for clients to get to and from the program via the Disabled Adult Transport System (DATS). We ask that clients please cancel their DATS rides if they aren't coming to the program on their scheduled days.
Attendance
On average, clients attend the program twice a week. Please try to schedule appointments on days you don't attend the program.
Discharge from the program
Clients are discharged from the program when the program team identifies that the client no longer meets admission criteria. Discharge plans are discussed with the client and family on a regular basis.

- Program manager/registered nurse
- Recreation therapy assistant
- Health care aides
- Physical therapist
- Occupational therapist
