We believe that the relationships between you and your health care providers should be built on respectful, open communication. This is essential to quality care. By sharing your feedback and concerns, we learn how we can better serve our patients, residents and their families.

Talk with your care team—they know your situation and will respond to your concerns.

Talk to the manager of the program or unit—you may also speak with the manager of the program, service or unit where you received care.

Covenant Health protects the privacy of individuals receiving health services in accordance with the Health Information Act. To properly review and resolve concerns, we work with the patient, resident or an authorized representative (usually a close family member), to gather and share information about the services received. Personal or health information collected will only be used for the purpose of tracking, follow up, communications and trending. Information is collected pursuant to section 33 of the Freedom of Information and Protection of Privacy Act and sections 1824 of the Health Information Act, as applicable.
